On the first day back to work after any period of sickness absence of up to 7 calendar days, you must complete a self-certification PDF form and email it to one of the Directors.
This form should be reviewed by a Director to confirm it has been completed correctly and the explanation given is medical in nature. The information will be held on your personal file thereafter.
